Appointment: $120/hour
Appointments are billed by the 1/4 hour.
Initial Consultation: 2 hours
Take a personalized comprehensive health history
Develop wellness goals which include dietary and lifestyle recommendations
(provided within one week after appointment)
Allow up to 2 follow up e-mails for Questions and Answers
Required at appointment
Record of 3-day diet
Copies of any recent laboratory tests
Bring any medications or supplements you are currently taking
Follow Up Consultation(s):
Review wellness goals and/or laboratory results

Live a long distance from Fenton, Michigan?
Appointments are available over
the phone. Recommended supplements or laboratory testing kits can be mailed
directly to your home. These appointments have worked out very well for
clients who live a long distance away or have difficulty with traveling.
Additional fees associated with these types of appointments include: shipping
and handling fees and long distance phone charges.
It is understandable that circumstances occur in life. The fee for simply not
showing up for an appointment is $35. If you have to change or cancel an
appointment please do so the previous day. Thank you.
Policies of Circle of Life Nutrition, LLC
Any laboratory results that are ordered through Circle of Life Nutrition, LLC will not be released prior to a follow up appointment. Should a client choose to have laboratory results that were ordered through Circle of Life Nutrition, LLC forwarded to another practitioner prior to that follow up appointment then there will be a $60 transfer fee.
Payment is due at the time of service. The client is responsible for all fees incurred. Each appointment is a separate occurrence and has an associated fee.
